Penile fibrosis, also known as Peyronie’s disease, is an penile disease characterised by fibrosis of the tunica albuginea, that is to say the poorly vascularised sheath that covers the cavernous bodies of the penis. This places restrictions on the elasticity of the penile area affected by the disorder during erections, consequently resulting in penile curvature.
The causes are unknown. However, it can be supposed that underlying the physiopathological process of the disease are repeated trauma or microtrauma affecting the erect penis, which cause lesions in the region of the tunica albuginea. The intervening normal reparatory processes of the body cause local inflammations and the formation of a scar: this is the typical “plaque” of Induratio Penis Plastica (I.P.P.), which can become impossible to modify over time, thereby stabilising the disease.
It primarily affects middle-aged men, only affecting young and elderly men very rarely.
In fact, in young men, the great elasticity of the tissue is able to absorb this type of trauma, unlike in the elderly, in whom poor tissue elasticity is however accompanied by a considerable reduction in energy during sexual relations: this explains why the disease occurs in middle age, when high levels of stress are still placed on the penis but the tissue gradually loses its elasticity.
I.P.P. has an acute phase and a stabilisation phase: it is during the acute phase, that in which the scarring processes are still underway, that the disease should be combatted with suitable treatment.
